Referring Physicians
We accept referrals from all across the mid-Missouri area. If you determine your patient is needing specialized care for their condition, one of our 26 board-certified fellowship-trained physicians would be happy to help your patient regain mobility and return to full health. The following are ways for you to refer your patients to Columbia Orthopaedic Group:
Web
You may fill out the Referral Form by clicking the icon below. This form will be immediately sent to our Appointment Group and processed. Please make sure you fill out your contact information and preferences so that we can get back to you and confirm the appointment. If you have demographic or clinical documents you would like to upload securely, we can accept any .pdf,.doc or .tif file. If you choose to communicate via email, you will receive a secure email from our office to confirm the appointment, or ask for additional information.
Fax
You may fax any documents to our Referring Physician Fax# (573) 876-8140. To make this process easier, you may wish to download our Referral Form by clicking on the icon below. This Adobe Acrobat form will allow you to fill out the form on your computer and print it out. You can fax this form, along with any other demographic or clinical documentation to the fax number listed above. Our Appointment Group monitors this fax line throughout the day.

You may call our Referring Physician Hotline at (573) 441-3759. We will be happy to take information over the phone and setup an appointment. Any documents necessary to setup the appointment may be faxed to our Referring Physician Fax# (573) 876-8140.
